Should your child require medication during school hours please download and complete a pupil medical request form.

Where possible the need for medication to be administered at school SHOULD be avoided - the staff cannot take responsibility for any dose that maybe missed.  Parents are requested to arrange the timings of the doses carefully.  EG 1 before school, 1 after school, 1 before bed = 3 doses.

Please note we can only accept and administer medication which is in the child’s name, prescribed by a pharmacist and in its original packaging.
